Our privacy policy ensures the confidentiality of your health records. Our practice management system allows us to securly keep accurate and detailed records which will never be sold to third parties in compliance with the Australian Privacy Principles (APPs) in the Privacy Act 1988.

• Personal information such as name, address, phone, date of birth, gender.
• Health history, cultural and lifestyle considerations.
• Occupation / employment details, interests, payment details, financial information.
• Information about how you found us or were referred to us.
• Health information including clinical and medical records.
• Family medical history and their details.
• Other medical service providers’ reports, diagnosis and test results.

Why we collect it:

• We collect these details to ensure we are delivering the best clinical service we can to our patients and as legally required from clinical notes.

• Details given over the phone or online to create bookings.
• Sign up form initial appointment.
• Referrals and/or findings from other services i.e. GP referral, x-rays, surgeons reports.
• Clinical notes, written from seeing patients, speaking with them over the phone or via email. 
• Health history, cultural and lifestyle considerations.
• From payments made.

Sharing of personal information:

• To comply with legal obligations.
• To consult with other providers outside of the clinic, however we will discuss this with you.
